The information on this page is relevant if you, or the person you are representing, is not registered with a GP practice in England and now live overseas.
Please note, the information on this page is for any of the following who are requesting access to medical records and patient details:
- Members of the public
- Solicitors and insurers
- NHS Bodies, Coroners, Police, Charities, researchers etc.

Subject Access Requests (SAR)
A Subject Access Request can only be made for yourself or the person you are representing is not currently registered with a GP practice in England.
Individuals should complete a SAR here to request access to medical records, and PCSE will process this request. Find out more about the process here.
Please complete the SAR form as fully as possible.
This option applies if you were formerly registered with a GP practice in England, and are now resident overseas. Responses to Subject Access Requests (SAR) can be sent to your new home address.
