Medical record requests for deceased patients

Overview

If you are looking to make a request for the GP medical records of a deceased individual, you will need to contact their last registered practice.

Please note, the information on this page is for any of the following who are requesting access to medical records and patient details:

  • Members of the public
  • Solicitors and insurers
  • NHS Bodies, Coroners, Police, Charities, researchers etc.

Patient registration information

Please click here to request patient registration information stored by PCSE (e.g. current GP, last GP, and GP registration history).

Last GP practice closed

PCSE will only process requests for deceased individuals whose last registered GP Practice is now closed.

If the GP Practice has closed, to make an Access to Health Records (AHR) Request click here. 

Information disclosure requests

PCSE will administer all Information Disclosure requests for deceased individuals from the following organisations:

  • Coroners
  • GPs
  • PHSO
  • Police

Any other organisations will be directed to contact the last registered practice of the patient, unless this is closed - in which case PCSE will process.
