GP practice requests

Overview 

PCSE process GP medical record access requests for living individuals who are not currently registered with a GP practice in England.

On this page, you will find information about requesting medical records for registered and deceased patients, as well as making patient registration / demographic information requests.

Medical records for registered patients

Patients registered at your practice

If you require the full GP medical record of a patient registered with your own practice, please complete a new Medical Records Movement enquiry using the dedicated form here. 

Please provide all relevant information about the patient, such as name, date of birth and NHS number.

If you believe the GP medical record for a patient is missing or incomplete, please click here for further guidance. 

Patients registered elsewhere

For access to health records of a patient who is currently registered with a different GP Practice, you must contact the GP Practice they are registered with. 

Medical record not received

If you are expecting a medical record and have not received it, you should submit a Medical Records Movement enquiry using the dedicated form.

If there is a clinically urgent need for the record, please you should submit an Urgent Medical Records enquiry. Find out more about Urgent Medical Record requests here. 

Medical records for deceased patients

Registered at your practice

If you require the full GP medical record of a deceased patient who was last registered at your practice,  please complete a new Medical Records Movement enquiry using the dedicated form here. 

Please provide all relevant information about the patient, such as name, date of birth and NHS number.

Registered elsewhere

If you are looking to make a request for the GP medical records of a deceased individual you will need to contact their last registered practice.

Please note, PCSE will only administer requests for deceased individuals whose last registered GP Practice is now closed.
